| 1. Purpose |
|
Carrying on the philosophy of the late Mr. Itoga, who continuously devoted himself to improving the welfare of the disabled, and promoting respect for the human rights of the disabled, the Kazuo Itoga Memorial Prize was established with the aim of realizing an integrated society that ensures the well-being of the disabled and their families. With this view, the Prize is awarded to those people who have contributed outstanding services toward the promotion of welfare for the disabled. |

Applicants' eligibility |
|
Individuals living in Japan, East Asia, Southeast Asia, and the Western
Pacific region (except for Australia and New Zealand), whose achievements
in welfare for the disabled are highly valued, and who are expected to
play even greater roles in this field in the future. |

Number of recipients and prize money |
|
In principle, the number of recipients shall not exceed two. |
|
Certificate and prize money of two million yen will be presented to each recipient. |
|

Please apply by "Application form for the 12th Annual Kazuo Itoga
Memorial Prize" (form＃1-1 or form＃2).(Either in English or Japanese.)

Neither recommending nor recommendation are asked. However, third party's
recommendation book (form＃1-2) is necessary in case of recommending person. |

A nomination by mail must reach the foundation by May 31, 2008. |

The application by E-mail is also enabled and let a term be the final
day of a collection period. |

Those who have been nominated previously but not chosen may be re-nominated |

| 6. Screening methods |
|
Recipients of the 12th annual Kazuo Itoga Memorial Prizes will be selected
by the Screening Committee from among the nominees（including nominees
for the previous three prizes), and their selection will be approved by
the Board of Directors |
|
| 7. Prize Presentation and winning memoril event |
|
It is scheduled to do in around November,2008.
